What is a sliding scale fee?
HealthCore Clinic accepts all patients, regardless of their insurance status or ability to pay. We are a Federally Qualified Health Center (FQHC), which means we can offer a wide variety of services through a sliding fee discount. The sliding fee discount is available to all income-eligible uninsured or underinsured patients and is applicable to most services. The discount is based on annual household income & family size. To qualify for the sliding fee discount, you must fill out an application and show proof of annual income for all immediate family members living in your household. Some of the accepted income verification materials are:
- Recent Federal Tax Return
- IRS Form W-2 or 1099
- Two (2) Current Pay Stubs

The sliding fee discount is in place to meet the needs of the uninsured or underinsured in our community by providing reduced costs on most services for those who qualify. All patients seeking care at HealthCore Clinic are eligible to receive services regardless of income or ability to pay.
